Meet Brittanny

A woman with curly dark hair, wearing a navy blazer and white shirt, standing outdoors with trees and a grassy park behind her.

I’m Brittanny, the founder of Blooming Transitions. With a background in nursing and a deep commitment to compassionate care, I support individuals and families navigating serious illness and the final stages of life with clarity, dignity, and presence.

My Background

My work as a nurse gave me a close view of how serious illness and end-of-life affect not only individuals, but the people who love them.

I often saw families trying to navigate difficult conversations, emotional overwhelm, and practical responsibilities without enough support or guidance.

Blooming Transitions was created to provide the kind of steady support that many families wish they had earlier… Someone to guide conversations, answer questions, and simply be present during one of life’s most important transitions.

My Approach

I believe that conversations about death deserve the same care and openness as conversations about birth, health, and living well.

When families are supported with information, presence, and compassion, this chapter of life can feel less frightening and more meaningful. My goal is to help individuals and families feel more supported, informed, and prepared during life’s most difficult transitions.

Serious illness and end-of-life experiences rarely affect just one person. They ripple through families, relationships, routines, and everyday life in ways many people are unprepared for.

Through this work, I’ve learned that people are often carrying fear, uncertainty, exhaustion, love, grief, and responsibility all at once.

Sometimes transitions unfold gradually. Others change suddenly. In either case, support, honest communication, and feeling informed can make difficult moments feel a little less overwhelming.

I believe people deserve space to process what is happening, ask difficult questions, and feel supported through moments that can feel unfamiliar or uncertain.

Kind Words

We are so grateful for you and what you did for grandpa and all of us.

I know that every family that you care for, you will touch their lives the way you touched ours, and that makes you a very special human.

— Ashley, Granddaughter of a Hospice Patient

“What makes her especially remarkable is the grace and presence she brings into difficult situations. The heart behind this work is real, and it shows in every interaction.”

— Lexus Clark, Former Healthcare Colleague

Support for wherever you are in the journey.

End-of-life experiences can feel overwhelming, emotional, and deeply unfamiliar for both individuals and the people who love them.

In the middle of changing roles, difficult conversations, and uncertainty, many families simply need space to pause, ask questions, process emotions, and feel heard without judgment.

My hope is to provide a steady presence through these moments while helping individuals and families feel more informed, grounded, and less alone.

You don’t have to carry this alone
